Medical Forms are required for all Scouts and Adults attending Scout events and are valid for 1 year. New forms should be turned into the pack once a year or when medical information needs to be updated. Only Parts A and B are needed for most events. Part C is only used for Scouts that will be attending an event that is over 72 hours, such as Resident Camp. All participating members of the pack (scout, mom, dad, siblings, etc.) are required to complete a medical form.  

Medical Forms for the pack will stay with the Pack for all pack events.  When attending Council Events you will need another medical form to present to the camp directors and can retrieve these forms after the event is over for future use.
